Tirzepatide is associated with several side effects, some of which may persist or emerge with long-term use. The most common long-term side effects include gastrointestinal (GI) symptoms such as nausea, diarrhea, constipation, and vomiting, which occur in a significant proportion of patients and often appear dose-dependent[5][6]. Rates for these GI events can range from approximately 40% to 50% at higher doses[5].
A small percentage of patients discontinue tirzepatide because of adverse effects, primarily GI-related, with the highest discontinuation rates seen at the highest approved dose[4][5]. Although most side effects are mild or moderate, more serious but rare adverse events can also occur—including acute pancreatitis, cholelithiasis (gallstones), and cholecystitis (gallbladder inflammation); however, clinical trial data suggest these occur in less than 1% of patients[4][5].
Tirzepatide and similar drugs have been associated with gastroparesis (delayed stomach emptying), which may become a chronic condition requiring long-term management[2]. Some patients have also reported symptoms suggestive of potential blood clots and deep vein thrombosis, though these risks are not officially listed on warning labels and further data are needed to clarify the association[2].
Rare cases of sudden kidney injury have been reported, typically in the context of severe vomiting or diarrhea causing dehydration[3][7]. Regular monitoring of kidney function may be recommended for patients at risk[3].
Animal studies reveal a possible increased risk of thyroid C-cell tumors with long-term tirzepatide exposure, though human studies have not confirmed this risk to date. Nevertheless, tirzepatide is not recommended for patients with a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia syndrome type 2[3].
Other rare but possible long-term psychiatric or metabolic effects (including mood changes, hypoglycemia, and vision changes) are reported, but these occur with low frequency and require clinical attention if they arise[1][7].
