Levetiracetam Side Effects

Levetiracetam is associated with a range of side effects. Common side effects include drowsiness, weakness, dizziness, irritability, headache, sleepiness, fatigue, aggression (especially in children), infection, and nasal congestion[1][2][3][6]. Gastrointestinal symptoms such as nausea, vomiting, diarrhea, and loss of appetite are also frequently reported[1].

Other commonly reported side effects are:

  • Loss of strength or energy
  • Muscle pain or weakness
  • Pain
  • Trouble sleeping
  • Joint pain
  • Tender, swollen glands in the neck
  • Voice changes
  • Body aches or pain
  • Congestion and sore throat

Some patients may also experience rapid mood changes, depression, irritability, aggression, anxiety, or personality changes[1][3][7]. Central nervous system effects can include memory loss, confusion, agitation, psychosis, hallucinations, and increased risk of suicidal thoughts or behavior[7].

Serious side effects, although uncommon, may occur and require immediate medical attention:

  • Severe behavioral changes (e.g., psychosis, hallucinations, suicidal thoughts)
  • Allergic reactions (including swelling of the face, lips, mouth, tongue, throat; difficulty breathing; rash or hives)
  • Serious skin reactions (e.g., blistering, peeling, severe rash)
  • Coordination difficulties (ataxia, abnormal gait)
  • Worsening of seizures, particularly if medication is stopped abruptly
  • Problems with kidney function (e.g., reduced urine output, swollen legs/ankles/feet, confusion, tiredness)

In rare cases, levetiracetam can cause anaphylaxis (life-threatening allergic reaction) and multiorgan hypersensitivity (DRESS syndrome), which require emergency care[2][4][5][6]. Speak to your doctor immediately if you develop a rash, swelling, tightness in the throat, significant changes in mood or behavior, or other worrying symptoms.
